Clients booking BODY CONTOURING SERVICES must complete BOTH forms A & B

Clients booking FACIAL SERVICES must complete BOTH forms A & C

• Form A – All Clients Required for ALL SERVICES – Covid 19 Policy – Consent & Liability Waiver – Optional Photo Release & Cancellation Policy
• Form B – Clients with BODY CONTOURING SERVICES – Body Contouring & Treatment Intake Form
• Form C – For all FACIAL SERVICES – Confidential Client Intake Form  

Completed forms are REQUIRED prior to your appointment. You can complete and submit them easily on any desk top or mobile device. If they are not completed in advance, then they will be required and be part of your scheduled appointment time for services. NO SERVICES WILL BE COMPLETED WITHOUT THE PROPER FORM COMPLETIONS.
